Community Habilitation & Individualized Support Services (ISS)
Community Habilitation
Our Community Habilitation program supports people in their home and community who either live independently in their own apartment or with members of their family. Community Habilitation plans are developed based on input from the person served and their circle of support. Once a plan is created, trained staff assist the person as they work to accomplish their goals. These outcomes include everything from budgeting, shopping, socialization, attending cooking classes, volunteering in the community, assisting with medical appointments, and maintaining their home. Community Habilitation is intended to supplement the natural supports from an individual's family and community and are not meant to be the sole source of support or oversight. Individuals typically receive several hours of this service each week.
Individualized Support Services (ISS)
EEDA serves people through ISS, an OPWDD housing subsidy that helps people pay their monthly rent or mortgage, certain utilities, and other expenses. Many of these individuals rent apartments in local complexes and a few of the people we support use this subsidy to pay their mortgage. ISS supports people who want to pursue a more independent living environment as a result of their personal growth and development. Many of the people in this program also receive Community Habilitation services to assist with managing their households and provide safety and risk management supports and services. This combination of financial allowance and support services allows people to strengthen their community supports and become independent members of the community.
