Supported Living

(ISS) Individual Support Services

This program provides financial support for individuals who desire to live in the community. In coordination with this service, residential habilitation staff ensures the necessary supports are in place to maximize independence. People receiving this service do not require intensive supervision, rather support services and periodic oversight. People in this program typically live alone or with friends or family in apartments in the community, many of which are located in the Riverhead area. Several individuals live with their spouses and children in homes we help them rent.

In one unique situation we have combined ISS resources to create a home for three developmentally disabled women who were pregnant and/or had small children, providing daily hands on parent training and supervision for their children utilizing a number of community resources.

We have been creative in meeting the housing needs of individuals we support through the ISS Program and have assisted two developmentally disabled senior citizens purchase their first and only home with the assistance of OPWDD's Home of Your Own project.

The Individualized support services program provides financial support to people who have the capacity to live independently or with limited staff support on the East End of Long Island. Participants sign a contract and agree to maintain a clean, safe and healthy home environment.
