Service Coordination: This program assists persons with developmental disabilities and their families and advocates, to gain access to necessary services and supports appropriate to the needs of the individual. Service Coordinators help in identifying the individual's aspirations, valued outcomes, and specify the services and supports necessary to achieve the highest quality of life. Individual choice, a person centered approach to planning and consumer satisfaction, are the key elements of good service coordination.
